Responsibilities
- Coordinate patient care plans and treatment schedules
- Maintain accurate electronic health records and documentation
- Liaise between patients, families, and healthcare providers
- Conduct patient intake assessments and eligibility verifications

Qualifications
- Associate's degree in healthcare or related field required
- 2+ years healthcare coordination or clinical support experience
- Strong communication and interpersonal skills

- Basic knowledge of HIPAA and healthcare regulations
