Responsibilities
- Provide direct patient care in assisted living and home health settings
- Administer medications and monitor vital signs per physician orders
- Maintain accurate electronic health records and documentation
- Coordinate care plans with interdisciplinary teams
- Educate patients and families on health management protocols
- Assist with mobility transfers and activities of daily living
- Respond to emergency situations following established protocols
Qualifications
- High school diploma or equivalent; CNA/HHA certification required
- Minimum 1 year healthcare experience preferred
- Current CPR/BLS certification
- Strong communication and interpersonal skills
- Ability to lift 50+ pounds and stand for extended periods
- Pass background check and drug screening
- Valid Virginia driver's license for travel between locations
